Hospice’s favourite ACP resources

Advance care planning (ACP) is a practice that is encouraged internationally, because it is so beneficial for patients, families and the health care team. So it makes sense that there are literally hundreds of helpful websites on the subject. We’ve narrowed down some of our favourite websites here to help guide you through the completion of an advance care plan.

  • The People’s Law School “Planning for your Future” guide offers easy-to-understand guidance on all legal matters, including Advance Care Planning. You can find information here regarding Representation Agreements and Substitute Decision Makers, Advance Directives, choosing your Power of Attorney, Elder Abuse, Estate planning, and so forth.

  • The BC Ministry of Health has established their own resources, including My Voice: Expressing My Wishes for Health Care Treatment”, updated in 2020. This document includes blank forms, including Representation Agreements. You can pick-up a print copy of My Voice at Hospice House in Davis Bay.

  • The Nidus website is an excellent legal resource for seniors and persons with disabilities, that walks you through many aspects of personal planning, from health to personal, legal or financial.

  • First Nations Health Authority has published a 2020 guide entitled, “Your Care, Your Choices: Planning in Advance for Medical Care,” which speaks to the importance of ACP for First Nations people.

  • Speak Up is Canada’s national ACP website which, “Provides the latest news, updates, activities and events from across the ACP community in Canada.” Established by the Canadian Hospice Palliative Care Association in 2008, it sets the tone for a national framework on ACP across the country.
